From NICU to Home: How We Support Families
BEFORE DISCHARGE
We encourage families to contact us while their baby is still in the NICU. Our team:
- Begins discharge planning early
- Reviews treatment plans and ongoing needs
- Coordinates GAPP or insurance approval so nursing can start quickly
- Creates a clear, realistic plan for going home
AFTER YOUR BABY ARRIVES HOME
Our nurses provide in-home support that may include:
- Daily clinical monitoring and safety checks
- Respiratory support, trach/vent/oxygen
- Feeding tube and pump support
- Medication administration and monitoring
- Early identification of red-flag symptoms
- Parent teaching and hands-on coaching
- Support around specialist visits and therapies
- Ongoing communication with your baby’s medical team
