Grace Pediatric Home Care is a premier, approved GAPP provider of in-home pediatric nursing for medically fragile children across Georgia. Our specialized nursing teams deliver hospital-level care at home 24 hours a day, 7 days a week for infants, children, and young adults with respiratory, feeding, neurological, and complex medical needs.

We partner directly with Georgia Medicaid, including Disability-Based Medicaid, Low-Income Medicaid, Katie Beckett/TEFRA, CareSource, Peach State, and Amerigroup, to ensure eligible families receive medically necessary care in the home.

Grace’s GAPP division includes geographically assigned clinical teams, strong hospital partnerships, and department-based medical services, allowing us to coordinate safe discharges, prevent readmissions, and support long-term stability for medically fragile children.

GAPP FAQs

What insurance covers GAPP?

We partner directly with Georgia Medicaid, including Disability-Based Medicaid, Low-Income Medicaid, Straight Medicaid, Peach State, Katie Beckett, CareSource, Amerigroup, and Peach Sate to ensure eligible families receive medically necessary care in the home.

Do we have to be in the hospital to apply?

No. Families may apply from home or hospital. Referrals are accepted from parents, physicians, case managers, and hospitals.

How long does GAPP approval take?

In most cases 30 days. Our team manages documentation, physician forms, Medicaid submissions, and follow-up to move the process forward as quickly as possible.

Can we change agencies if we already receive GAPP services?

Yes. Families may transfer. We coordinate a smooth, safe transition.

What happens when my child turns 21?

GAPP ends at age 21. We help explore adult Medicaid waiver programs and plan the transition.
